Check-in Instructions and Patient Forms
Please arrive to your appointment 10 minutes early, if possible, to have time to check in and fill out necessary form(s). To expedite your service and minimize your in-office wait time, please download and fill out the patient check-in form and bring a printed copy to your appointment. Forms are available for download below.
New patients who already wear contacts are asked to bring any information about their current contact lenses (Signed Rx, old contact lens boxes, etc). This will aid the doctor when the contact lens evaluation is performed.
